What happens after I report the theft?

After you report the theft, you will be asked to complete an Online Lost Benefit Attestation Form if you have not submitted one already.  

 

After MDHS receives your attestation form, an investigator will review your claim and either approve or deny the claim within 10 days of the receipt of the attestation form. 

 

Once a decision has been made, you will receive an approval or denial notice. 

 

If your claim is approved, you will receive your replacement benefits on your EBT card within 10 days of the claim approval.
