Supporting Documents

The CCPP Application requires a few supplemental documents that should be included with your application. These are:

Send All Materials to DECCD

Second, send your application and supporting documentation to DECCD. When your application is processed, you will receive a confirmation email with your Provider ID. You can apply three different ways:

  1. Via our online portal, Service Request
  2. By fax at (601) 359-4422

Register for eLedger Training

Attend a provider e-Ledger training webinar (Online Training) held on the first and third Tuesday of every month at 12:30 p.m. You must register in advance and attend the entire session to receive credit for attendance.
